- For maximum efficiency, register online at www.WesternRegional.org
- "Early Bird" registration closes March 11, 2008. Save time and money by registering online or by mail by March 11th AND receive your convention materials (badges, tickets, etc.) in the mail prior to the convention! Then, just bring them with you and start your adventure!
- Those who register after March 11, 2008 will be required to pick up convention materials on-site at Phoenix Convention Center.
- Pre-registration (online and mail) will close March 28, 2008. Those who have not registered by this date will be required to register on-site at Phoenix Convention Center. On-site registration is strongly discouraged.
- Seating at all Open Lectures will be on a first come, first-served basis.
- The Western Regional Dental Convention is a great value! The average cost of attending is $8 per CE credit hour.
"Early Bird" Registration.
Register by March 11, 2008 to receive registration materials & badge(s) via mail prior to the convention. Materials will be mailed to the PRIMARY registrant. Failure to bring packet to convention will result in a $10.00 per person re-issue fee. Registrations received online or postmarked after February 11 must be picked up at convention.
Refunds/Cancellations
for registration and/or tickets may be obtained ONLY by written request accompanied by all badges and tickets and postmarked by March 21, 2008. Refunds requested after March 21, 2008 will not be granted, including requests made based on absence due to late arrival for courses, weather and/or parking difficulties. A $10.00 processing fee will be deducted from all refunds. Refunds will be processed and mailed by April 30, 2008.
Registration Policies.
Attendees over age 18 must register. Persons under age 18 are permitted in exhibit hall ONLY. Dentists must register as dentists. AzDA does not issue Guest badges for the Western Regional Dental Convention. Non-AzDA member dentists must present ADA card to receive member rate. Convention badge entitles entrance to open lectures and exhibit hall, and eligibility to purchase tickets for workshops and events. Spouse/Family not permitted to attend lectures or workshops.
Payment Policies
All credit card transactions are processed in U.S. Dollars and are subject to the current exchange rates. Returned checks are subject to a $25.00 fee.

Classroom Policies
Classroom seating is available on a first come, first-served basis. Cellular phones and pagers must be turned off or placed in "silent mode" prior to entering classrooms. Spouse/Family and children not permitted in classrooms.
Lunch
AzDA is offering Box Lunch Thursday, Friday and Saturday for $15.00 per person per day. Must be pre-purchased with convention registration (use registration form). Box Lunch Tickets will be mailed with registration materials to primary registrant indicated on registration form. Box lunches will NOT be available for purchase on-site during convention. You MUST pre-order. Deadline to order box lunch is March 28, 2008. NOTE: Ticket must be presented to pick up box lunch each day. No exceptions. Replacement tickets will NOT be issued.
Food at Convention Center
Light fare and beverages may be purchased from the Phoenix Convention Center in the rear of the exhibit hall.
Concierge Desk
Located in the Phoenix Convention Center lobby, representatives will be available to direct you to area attractions and activities such as dining and golf.
CE Credits
The number of CE credits is equal to the number of course hours. CE verification forms will be included in the Official Convention Guide, provided to all attendees at the convention. Verification codes will be announced at the conclusion of each CE credit course. Full-day courses will have two codes -- one for morning session, one for afternoon session. Codes will not be given any other time. Documentation of verification codes is attendees’ responsibility. NOTE: Verification codes are NOT required for CPR Certification/Recertification Workshops.
CERP
The Arizona Dental Association is a Continuing Education Recognition Program provider conducted under the auspices of the American Dental Association.
AGD Credit
For validation, the Arizona Academy of General Dentistry will be located in our Exhibit Hall in Booth #134.

A special group rate of $191.00/night plus applicable taxes for single/double has been arranged for participants of the Western Regional Dental Convention. For additional persons, add $25/person to the above. Special Group Rate will be valid through March 13, 2008 only.
Hotel Reservation Form is included in this booklet. Form must be faxed to (480) 344-1442 or mailed to AzDA at the address below. Hotel will not accept reservation form directly, HOWEVER, you may telephone the hotel directly at (602) 252-1234 to make your reservation. Additionally, you may utilize the services of Central Reservations toll-free at (888) 421-1442. When making reservations using ANY of the above methods, be sure to reference “Arizona Dental Association” or “2008 Western Regional Dental Convention” for maximum value.
Frequently Asked Questions
How will I track my CE's?
At the conclusion of each lecture and workshop, a code will be provided verbally. It will be your responsibility to record the code in the space provided in your convention program guide. It's important that you maintain your CE credits while at the convention, and retain your notes, as no certificates will be issued. For AGD members, visit the AGD booth #1032 to obtain your verification.
How can I register?
Online registration is strongly recommended. You may also register by mail. Attendees whose mail-in registrations are postmarked by March 11, 2008 will receive their materials (badges, etc.) prior to the convention and will be responsible for bringing those materials to the convention.
On-site registration is strongly discouraged, as it may delay your convention adventure. If you must register on-site, arrive early. Although AzDA makes every effort to make on-site registration as smooth and efficient as possible, waiting time can average one hour or more. Pre-registration will officially close March 28, 2008.
What is the difference between an Open Lecture and Workshop?
Open Lecture seating is on a first come, first-served basis and the only entry requirement is your official convention badge. No tickets or passes are issued for Open Lectures.
Workshops involve hands-on instruction, typically requiring tools and/or equipment. Attendance at a workshop requires a ticket purchase, and seating is typically limited. Again, register and purchase workshop tickets early!
May I bring spouse, children, guests to the convention?
Attendees registered as Spouse/Family will be issued a convention badge that restricts attendance to the Exhibit Hall only. The Arizona Dental Association does not issue "Guest" badges for the Western Regional Dental Convention. Spouse/Family and children not permitted in classrooms.
Will I receive a confirmation if I register online?
YES! Upon completion of your online registration process, your confirmation will be sent to the email address you provide.
When will I receive my badge?
Attendees who register online or whose mail-in registrations are postmarked by February 11, 2008 will receive their materials (badges, etc.) prior to the convention and will be responsible for bringing those materials to the convention. Convention materials will be mailed to those who pre-register beginning in mid-January.
Those who register online or by mail after March 11, 2008 will be required to pick up their materials at the Pre-Registration Booth located in the Phoenix Convention Center Lobby Registration Area.
What should I do if I registered but can't come?
Request for refunds must be in writing and postmarked no later than March 21, 2008. If you received your convention materials (badges, tickets, etc.) in the mail, you must include them with your written cancellation.

I don't wish to take any courses but I'd like to visit the exhibit hall. Is that an option?
Yes! You may purchase a one-day, Saturday-only exhibit hall badge. When registering online or by mail, choose tuition code Z.
I work for an AzDA member dentist. Does that automatically make me an Allied Member?
No. Dental Hygienists, Dental Assistants, Dental Laboratory Technicians and Dental Front Office Staff employed by active or life members of AzDA are eligible to apply for Allied Membership. Visit this page to learn more about Allied Membership.
What is the convention dress code?
Business casual is suggested and layers are IMPORTANT. Some classrooms may be cold, some warm, some just right - it's best to arrive prepared. Most of all, wear comfortable shoes. Our exhibit hall covers 100,000 square feet!
Will lunch be provided?
On Friday, don’t miss our Dental Team Luncheon, featuring an elegant meal and professional education and entertainment and (1) CE credit hour in Practice Management! Purchase tickets early! Seating is limited and seats go fast!

Thank you for sending my registration materials, badge and tickets in the mail. Unfortunately, I spilled coffee and ruined everything! What should I do?
You have two (2) options: Call AzDA (480-344-5777 or 800-866-2732) by March 21, 2008 to request replacement materials and then pick them up at the convention. If you are unable to call by March 21, you will be required to order replacement materials at the convention, and for this option, there is a $10.00 per person fee. Workshop and/or Special Events ticket(s) MUST be returned, even if damaged, in order to receive replacement(s).
