All lectures and workshops are "ticketed." Please pre-register for any lectures, workshops or special events you are interested in attending. Lectures are no longer open; you must pre-register (and arrive on time) to secure a seat.

I'm trying to register online and it won't allow me to pay.

You must select your courses first before the system will allow you to complete your registration and pay. Pre-registration is required this year.

I'm not an AzDA member dentist, but would like to become one as soon as possible so I can take advantage of the member rate for the convention. What do I do?

The dentist I work for is an AzDA member. Does this automatically make me an AzDA Allied Member?

No. You would only currently be an AzDA Allied Member if you applied for membership and paid your $40 dues for 2012. Contact our membership department for questions at (480) 344-5777 x302 or download an Allied applicaiton to join.

I thought I was a current AzDA Allied Member, but my membership number is not working when I am attempting to register online.

I don't feel comfortable registering online. How else can I register?

You can complete part the of registration online, and at the payment area select "Pay By Check." This will allow you the option to choose your courses and mail in your registration with a check. Or, you can simply download the "By Mail" form below, fill it out, and mail it in with either a check or credit card information to:

If you choose this registration method, please make a note of the sold out courses before you write your course selections on the form. Remember that pre-registration for courses is required this year, so you must write in your course selections on the form prior to mailing it in. Also, please note that all courses you register for on Thursday must have a "T" course code, all Friday courses must have an "F" course code, and all Saturday courses must have an "S" code.

Refunds/Cancellations
Refunds for registration and/or tickets may be obtained ONLY by written request and must be accompanied by all badges and tickets and must be postmarked by March 23, 2012. Refunds requested after March 23, 2012 will not be granted, including requests made based on absence due to late arrival for courses, weather, and/or parking difficulties. A $25.00 processing fee will be deducted from all refunds. Refunds will be processed and mailed by April 30, 2012.
