These contract conditions, rules and regulations are part of the 2010 Sponsorship Application and are binding for participating companies. Accordingly, all participants should read these rules carefully before signing the Application



Processing of sponsorship applications
AzDA accepts for processing only those applications that: (a) include all required information; (b) are signed by an appropriate and authorized representative of sponsoring company; (c) are accompanied by the required payment. AzDA reserves the right to exercise its sole discretion in the acceptance or refusal of applications.

Contract is non-cancelable
This Application serves as a contract and is non-cancelable. Any revisions or modifications must be agreed to by both parties in writing.

Advance reservations for WRDC 2010
Companies may reserve in advance sponsorship items for the following year (2011) by signing a letter of intent and submitting it to AzDA with a 50% deposit. This commitment will be bound by these Rules and Regulations and subject to a cancellation fee.

Deadlines

• Benefits of sponsorship are subject to print deadlines as defined by each individual benefit..
• Sponsorship must be secured prior to published deadlines to receive all assigned benefits.

Availability of Benefits
Benefits are subject to availability based on time of year sponsorships are purchased and material deadlines. In addition, benefits are only available in 2009/2010 and are not transferable to 2011, or redeemable for cash or other benefits.

Sponsorships of $3,000 or less
In order to be eligible for sponsorships of $3,000 or less at the 2010 Western Regional Dental Convention, sponsoring company must purchase a minimum of one (1) fair price exhibit booth at the 2010 event or be a current AzDA Services company.

Agreement and restrictions
These Rules and Regulations serve as the contract conditions and agreement between AzDA and the Sponsor identified on the Application for the purpose of securing a defined sponsorship for the 2010 Western Regional Dental Convention. The sponsorship description, benefit and cost are defined in the attached sponsorship literature; no other benefits are guaranteed or implied. The sponsorship is non-transferable and any form of resale is prohibited. The Application will serve as a valid booking order for determining priority placement. In the event that the sponsor is authorized by AzDA to acquire or provide goods or services in lieu of part or all of the sponsor payment, a detailed description of the product or services and their fair market value must be submitted in writing to AzDA for approval. AzDA reserves the right to reject any goods or services which in its own estimation do not meet its sponsorship revenue or value objectives. Any person executing this agreement represents that he or she is fully authorized to do so and agrees to be bound to all terms and conditions by signing the Application.
